CarePlus Mobile Health makes managing your health simple.

To get started, visit the registration page

• Enter your first name, last name, last four digits of your Social Security number, and your date of birth in a MM/DD/YYYY format
• Select Register
• Enter a password and confirm it
• Choose Next and you’ll be redirected to the login page
• Sign in using the username and password you created

Once you've created an account, you'll access the site in the future by going to CarePlusMobileHealth.com and signing in with your email username and password.

Your employer, or your spouse/domestic partner’s employer if you medically enrolled in their benefits, is providing cash incentives for participating in your well-being program. CarePlus Mobile Health is the entry point into these programs. It also provides valuable information on other programs available to you and your medically-enrolled family members.

CarePlus Mobile Health makes it easy to access your well-being programs—including access to your personal health information, incentive rewards, and healthy activities.

CarePlus Mobile Health is provided by Mobile Health Consumer (MHC). MHC collects information in order to provide suggestions on benefits available to you through their secure platform. Your name and your eligibility for services is provided by your Employer, or your Spouse/Domestic Partner's employer. MHC may use health claims or biometric data from your health plan, insurance provider, or a third-party vendor to provide available services.

Mobile Health Consumer uses the information you provide or that it collects in order to establish and enhance its relationship with you. Mobile Health Consumer generally uses such information to operate, maintain, enhance, and provide all of the features and services found in the Services section.

Mobile Health Consumer may use the information you provide or that it collects in order to periodically provide reminders about posting or submitting new data or content to your healthcare providers through Services. Mobile Health Consumer may also use your personally identifiable information to provide information, newsletters or offers about Mobile Health Consumer's features and services that may be of interest to you.

Mobile Health Consumer uses all of the information you provide or that it collects from Users to understand and analyze the usage trends and preferences of its Users, to improve the look and functionality of Services, and to create new features.

Mobile Health Consumer may use "cookies" information to: (a) remember some of your information so that you will not have to re-enter it during your visit or the next time you visit the Website; and (b) monitor aggregate site usage metrics such as total number of visitors and pages viewed.
